This is the form you have to fill in if you want to be part of the book. It sets out the rules and you filling it out tells me that you agree to abide by the rules of the book and all times

1: No duplicates of a character to make it easier for me to tag you when you have a question

2: Be respectful to everyone who asks a question but you have the right to refuse to answer a question if you aren't comfortable answering it

3: Answer the question honestly

4: Please inform me if you decide you don't want to remain in the book

(Please copy the rules out and put a tick beside them so I can see that you agree to follow them)

The consequences of not following the rules are..

1: A warning that you need to follow them

2: A temporary suspension of your role in the book

3: A permanent suspension of your role in the book and you will be replaced by someone else

State your character's first and Last name in block capitals to state that you agree to abide by the rules and you understand the consequences of not following them.
